Osteosarcoma commonly metastasize to the lung, lymph nodes, liver and brain. In this case we describe a rare presentation of direct infiltration, by an osteosarcoma, of the iliac veins that continued into the inferior vena cava (IVC)and could be seen up to the right atrium.
